The Service Record Request Form is the required document for all former and existing Greenville ISD employees to request an official service record from the District. Please complete and submit the form online.

The Exit Interview/Survey Form is the required document for all current Greenville ISD employees to complete upon the end of their service with Greenville ISD. Please provide feedback on your experience with Greenville ISD.
The Transfer Form is to be used Internally by Greenville ISD Administrators. Once complete, you will be notified by Human Resources that your form has been received and being processed.
The updated GISD Travel Form is to be used by Greenville ISD staff when requesting travel reimbursements. Once complete, the form will need to be submitted to your Principal/Supervisor.
The updated GISD Conflict of Interest Form is to be used by Greenville ISD staff to disclose nonschool employment. Once complete, the form will need to be submitted to your Principal/Supervisor.
All employees leaving the district will need to submit the GISD Resignation/Retirement form to the HR Department. This form requires Principal/Supervisor approval. Contract employees will need Superintendent approval as well.
